Survey:

I've made all the changes and additions noted and turned on the logging.
I would appreciate it if people on this list would test it out, either
indicating a test or an actual submission, so we can publish the URL in
Wednesday's mailing and get some results in before the holiday weekend.

Hmmmm ... I should probably change the form method from GET to POST,
since the volume of the data is significant. That's an easy fix and will
prevent some submission lockups. It's also necessary when we have large
text areas such as what you suggested below.

We definitely do not want any lockups when people take 5 minutes to give
us this data.

Not the way it's designed. I would hope nobody had any great interest in
skewing our results.

I was planning to post and analyze the results right here on the org
list. Maybe a general summary in the main list as well. We could
generate all kinds of pretty graphs, like miles traveled vs. dollars
spent, if there is a need for it.
